I gather that the writer assumed when he submitted the script someone would say no... and nobody did.

It actually felt as though the whole thing had been written with a different ending in mind and 7/8ths of it had been filmed when the phone rang and someone said to the director: I'm sorry but you have to kill them all off... right now. We're not going to a fifth series. So the director walked back into the studio and said: Right, lads, this is what we're gong to do...
